Calendar Year Chapters must complete all of the following required Elevation forms for the August 30, 2024 deadline.

- Finance & Facility Form (includes live-in roster if applicable)
- Roster Update
- Contact Form
- Hazing Compliance Form
- Notice of Membership Intake/Recruitment Intent
- Priority Area Action Plan Revisions
- Web Update Form
