Risk Management & Emergency Procedures Form

This form requires the submission of two separate files (unless the items are already combined in one document).

  1. Risk Management Policy: Your organization should have a risk management policy locally developed or provided as standard by your (inter)national organization.
  2. Emergency Procedures: Organizations should have procedures or protocol to enact in case of emergency. These may be developed locally or be provided by the (inter)national organization. These procedures typically provide insight about responding and how to triage emergency situations, communication structures, and who can represent the organization in a time of emergency. You can view the OFSL provided Emergency Response Plan Template for your use in completing this part of the form.

The risk management policy uploaded here is your organization’s policy. Chapters are responsible for knowing their own policy as well as the Fraternity & Sorority Joint Policy on Risk Management. To assist chapters in comparing both policies, we encourage chapters to utilize our risk management policy comparison tool. Your chapter liaison or council risk manager can assist in helping you through this document if you do not know how to identify an answer to the questions contained in the tool.

Emergency procedures should include your chapter's plan for what to do in case of an emergency. This often includes a communication chain as well as information about who should be notified and who may provide comment on behalf of the chapter. Many (inter)national organizations provide guidance or templates for emergency procedures, so it is recommended you connect with your organization before crafting something new.
If your risk management policies are in the document attached in the last question, please upload the same document a second time. A risk management document includes risk management related policies and expectations (most often provided by your (inter)national organization). Typically, this is on a wide range of risk-related topics including alcohol use, social event planning, hazing prevention, fire safety, sexual misconduct, etc.
