MGC Notice of Intake Form

This form collects recruitment and intake-related information. If you intend to bring in new members this semester, you will be asked to provide information about your informational events (including date, time, location, etc.). As well as information about your intake program  (including all activities held as part of the program).

For this reason, we encourage chapter officers responsible for all elements of the intake process to work together to complete this form. This might also require preparing to submit this form by gathering appropriate documents for upload.
