Hazing Compliance Form

The chapter president is responsible for reading the Colorado State University hazing policy from the Student Conduct Code (located here), the State of Colorado Hazing Law (located here), and the hazing policy within the Fraternity & Sorority Life Joint Risk Management Policy (Located here) to the general chapter membership annually. The chapter president is also responsible for reading the statements within this hazing compliance form to the general chapter membership. After both of these tasks are complete, the chapter president should certify they understand the information indicated here by completing the following form.

  • We certify that all activities sponsored or required by our organization comply with our inter/national organization’s hazing policy.
  • We certify that all activities sponsored or required by our organization comply with the state of Colorado Hazing Law, CSU hazing policy as outlined in the Student Conduct Code, and the hazing policy within the Fraternity & Sorority Life Joint Risk Management Policy.
  • We understand that new members, initiated members, and alumni members are prohibited from participating in hazing activities, as defined by (inter)national organizational policies, university policies, the hazing policy within the Fraternity & Sorority Life Joint Risk Management Policy, and the state of Colorado, and the chapter can be held responsible for the behavior of all above mentioned members.
  • We understand that no membership intake and/or membership education activities should occur that do not comply with the process outlined by the (inter)national organization.
  • We understand that the chapter is not to engage in any pre or post pledging/new member activities that do not comply with the process outlined by the inter/national organization.
  • We agree to inform interested students, new members, initiated members, and alumni members of the (inter)national organizational policies, university policies, the hazing policy within the Fraternity & Sorority Life Joint Risk Management Policy, and the state of Colorado law related to hazing.
  • We understand that violating hazing policies will result in a referral to the Student Resolution Center and/or the Joint Judicial Board for the chapter and/or those individuals involved in the hazing activity.
  • We understand that our (inter)national headquarters will be notified if there are any concerns or allegations of hazing occurring within membership activities.
  • We understand that participation in any hazing activities or the failure of our organization/individual members to take necessary steps to stop the hazing from occurring may cause personal referral to the Student Resolution Center. Further, we understand that having prior knowledge of a hazing event and not taking steps to prevent the hazing event may also cause personal referral to the Student Resolution Center.
